Requesting Copies of Your Medical Records
To obtain copies of your medical records from BryanLGH Medical Center you may print the authorization form thoroughly complete it, and either fax it to 402-481-8769 or mail to:

If you have questions or would like a copy of the authorization form mailed to you, please call the release of information section in the health information management department at 402-481-8424 Monday through Friday 8 a.m.-5 p.m.
Requested copies will be mailed to you in approximately five business days or you may pick them up at the above address at a predetermined time.
